Required Documents

Documents you need to bring to the Passport Application Centre (PAC) for your biometrics appointment

Important

All applicants must bring original documents to the PAC on their appointment date. Photocopies alone are not accepted. Ensure all documents are clear, legible, and up to date.

Minors & Parental Consent

For applicants under 18 years of age, parents or legal guardians must accompany the minor to their appointment. Additional documentation is required.

When is the consent form required?

The parental consent form is required when a parent or legal guardian is not accompanying the minor to the passport office. For example, if parents are divorced and only one parent is applying on behalf of the child.

When is it not required?

The consent form is not required if both parents attend the passport office in person with their child.

Guarantor Requirements

Two (2) guarantors are required:

LiabilityDeemed to have agreed jointly and severally to pay all expenses incurred by the government on the applicant in the event of repatriation or death abroad

Witness Requirements

One (1) professional witness is required. Acceptable professions include: senior clergy, commissioned military officers (Captain+), senior police/immigration/customs/fire officers, senior civil/public servants, registered medical practitioners, solicitors or barristers, heads of recognized educational institutions, and other registered professionals.
